With this document, also referred to as a “Living Will,” “Power of Attorney for Health Care,” or “Directive to Physician,” an individual makes choices regarding medical care for situations in which he or she might become incapacitated. In a Health Care Directive, you can also state your wishes regarding extraordinary or life-sustaining treatment, donation of organs and disposition of your remains at your death. One or more individuals can be designated to make decisions in the event you are not able to do so.
